Transition Manager paying £32kpa
9am-5pm Monday to Friday
Reporting to the Registered Manager, the Transition Support Manager will lead on the development of our supported living infrastructure in preparation for the transition to supported living accommodation and service for people with a range of disabilities.
The purpose of the role is to support the Registered Manager in ensuring compliance of the service as required by Care Quality Commission (CQC)
What You Will Be Doing
Operational
·Facilitate the development of an operation manual for the service inlcuding working with key stakeholders, such as housing, hr, training, health and safety and maintenance teams to ensure the documentation of process and development of policies.
·Work closely with the Registered Manager in the delivery of high standards of quality and safety by developing a strong audit and oversight regime.
·Work with the care teams to map out and document current processes, and identify efficiencies.
·Consult and liaise with a wide range of stakeholders including, relatives, friends and other professionals, both current and prospective.
·Liaise with all other corporate teams to ensure that best practice and information flow is efficient and effective.
·Work with our marketing team to develop materials to promote the services of the client to external stakeholders to create new business.
Quality
·Measure staff engagement with the Care competency framework to support the development of the supported living services. Identifying and developing solutions to any barriers that may be in place.
·Ensure compliance with CQC and best practice delivering an approachable, credible, value-added point of compliance matters relevant to a Supported Living Service.
·Ensure all staff members are fully conversant with the CQC requirements and the role they play in an inspection.
·Support where required, the care planning process supporting service users with their plans, ensuring a person-centred approach is applied in all cases.
·Carry out quality reviews of service, ensuring that all areas of improvement or non-compliance are identified and acted upon in a robust, timely and effective manner.
·Work with the team and the Registered Manager to develop, maintain and deliver a continuous improvement plan for the service.
·Ensure all environmental and health and safety checks are carried out in line with regulatory and Trust requirements.
 
Leadership
·Motivate and manage staff to ensure they achieve their potential in line with the clients policies and procedures.
·Review the quality of support plans and risk assessments, working with staff to improve where required.
·Demonstrate leadership and act as a positive role model inline with the Clients competency framework and core values.
·Assist in the recruitment, interview and induction of new Support Workers.
·Actively collaborate and engage with other functions and initiatives across the Trust.
·Willingly engage with any other duties and responsibilities that may be required from time to time.
